Information for new patients

Diabetic patient

14 days prior to the initial examination, the patient will attend the application of the FreeStyle Libre sensor, bring a completed downloaded questionnaire and completed minimum of 3 days ideally 7 days dietary record form (stating the weight in grams), the patient will also receive a referral for blood tests.

*If the patient is unable to download the menu and questionnaire, they will receive both with the application form and bring them with them to their first appointment.

Endocrinological patient

Before the first initial examination it is necessary for endocrinology patients to collect a blood test referral (or it is possible to send an electronic referral to the following laboratories - Česká laboratorní, Prevedig, MeDila, Spadia, Unilabs, Vidia.

Requesting an ePrescription

Please use our online form to request an ePrescription for your regularly used medications prescribed in our DIABETOLOGY/ ENDOCRINOLOGY clinic.

Please note that:
- you need to enter the name of the medication including its strength (e.g. Metformin1000 mg etc.).
- it is necessary to request an ePrescription well in advance.
- patients who have not had a follow-up appointment for a long time will not be sent an ePrescription. In this case, we will inform you about the further steps.
- you will only be sent the medication that has been prescribed by our doctor.
